Independent Contractor Agreements

To request an Independent Contractor Agreement please complete the form below:

You will be required to attach the following documents:

  1. Current Business License
  2. A copy of your 2026 W-9
  3. Quote for services (this is not an invoice)
  4. A Certificate of Insurance showing a single combined liability limit of $1,000,000 and naming as ADDITIONAL INSURED: "Lucia Mar Unified School District, its Board, officers, employees, agents, volunteers and invitees."

If the scope of your work involves frequent or prolonged presence on campus and/or around students, or any potential interaction with a student (or students) outside of the immediate supervision and control of the student's parent/guardian or school employee, you will be required to provide these documents for each employee who will be working around our students:

  1. DOJ background check via Live Scan/Fingerprinting
  2. Certificate of Completion - Child Abuse Mandated Reporter Training
  3. TB Risk Assessment - current as of the last 60 days (required only if the scope of your work requires frequent or prolonged contact with students)

You will be informed if these additional requirements are needed.

Completing this form does not constitute an agreement. Services will not be agreed upon and payment will not be provided without a fully executed Independent Contractor Agreement.

Once it has been determined that these minimum qualifications are met and the school site/department has confirmed that services are desired, you will receive an Independent Contractor Agreement at the email address provided to be signed electronically via DocHub. 

AFTER services have been provided, please send an invoice to your contact at the school site or the department you are working with so that they may initiate payment.
